When I have my foot on the brake of my car or reversing my car is jerking Whats this from?

I have a 98 GMC Suburban and sometimes when I start it and reverse out of the driveway it jerks backward.Also when I'm in drive but have my foot on the brake it jerks forward. And its been dying and jerking when I try to take off from the stop light. Changed the fuel filter Does anyone know what this could be?
